There is a need to be able to set a table of contents entry for a clause
to be different than the title of the clause.
For example:
The clause might be:
8.4.12 Logical Block Size (BP 129-132)
However, someone may want only the following in the TOC:
8.4.12 Logical Block Size
This would seem to be doable by adding an argument that, if present, is the 
title to be included in the TOC.
