|  |     re: 37.1 -- This is definitely the same problem. If a PREFACE is
    included, its first page number is not printed; if no PREFACE, the
    first page number is omitted from the first chapter.
    
    Chased this "bug" around today, and found that it appears only if
    the FRONT_MATTER includes a <CONTENTS_FILE> tag. Omit the tag and
    the probelm goes away.

    This is the front half of the problem noted in the Release Notes
    (extra page when <index_file> is specified). I would like to know
    what doctype(s) are in question here, so I can double-check that
    we've fixed.

    I was just about to enter a note on the same subject.
    
    With the General Doctype, a <running_title>(title\THIS_PAGE) will
    not produce the title on THIS_PAGE if the page has a <CHAPTER> tag
    on it.
