Has anyone experience this? When using the <span> tag with the
<table_head> tag does the header that is supposed to center
across x number of columns appear non-centered, more to the
right? The hardcopy version is correct but when building the
book for online version the headers do not centered over
the columns correctly. Any solutions and or explanations?

Yes, I had that problem, although the hardcopy version was also
slightly off-center.
I discovered that the documentation for the <SPAN> tag discusses this
problem. It suggests that you code the table with an additional column
and specify a width for that additional column.
