Erratic access to some index entries that have multiple choices


Some, BUT NOT ALL, of my index entries with multiple references are 
not giving the window with index choices.

In the sample below, the items marked "x" click ONLY to the FIRST 
ENTRY.  They do not give a window with choices.   Some entries, 
including those with NO "x" below, do give the appropriate window with 
choices for each spot in the book.

Other Info:  There are no "spaces" in the .sdml file around these
index hits. We are viewing books built in Bookreader V1 through the
Bookreader V2.

    What version of the online tools was this book built under? This
    sounds like a problem that we found in the Bookreader V1 tools and 
    fixed for the V2 tools. Please rebuild the book under the V2 tools
    and let me know if the problems remain.
