I seem to have been hit with a cosmic ray.
When I select the book icon for my entire book and select print
->printerleaf -> document, Interleaf creates 3 printerleaf files. One
contains the TOC through Chapter 6, one contains Appendix A, and the
last contains the glossary. I'm not sure where the index fits in.
This is the layout of the icons when I first started.

cmdappendix used to
be here.
First, I tried to solve it by selecting all of the sub books and
selecting book->line up and book->sync. The books all lined up at the
top of the window, and I selected print->printerleaf->document.
Three books again.
So then I tried to create the command appendix again. and try it that
way.
Three books again.
What happened?
How can I get Interleaf to create one printerleaf file for my whole
book?

When this has happened to me, I had different page sizes. After I made
the page sizes the same, I was able to generate one complete file.

Question:
I'm trying to make a Printerleaf file so that I can make a postscript
file of a book. It makes separate Printerleaf files. What am I doing
wrong<? How do I fix it?
Answer:
Not all of your documents within the book have the same page
properties. All documents must have the same page properties to make
one Printerleaf file.

Question: When I try to create a PostScript file from a Printerleaf
icon, nothing comes of it. I am clicking on the Printerleaf icon and
selecting Custom > Filter > POSTSCRIPT. What's going on?
Answer: There is one more level to select:
Custom > Filter > POSTSCRIPT > LN03R_PS.
Question: But Steve, Interleaf uses default menu selections! The
default *is* LN03R_PS. Why do I have to slide all the way over?
Answer: Ah, grasshopper, indeed that is the default, but Interleaf is
not always consistent about applying defaults. Why do you think
nothing is happening for you?
