    is standard with ALL-IN-1 IOS...

    This was issued as a separately-orderable book, part number
    AA-PKUCA-TE.
    
    I couldn't find an original version with the other documents in our
    LIBRARY account, so I've just built one.  I can't guarantee that this
    is exactly the same as the book that you can buy, and I haven't checked
    all 302 pages, but it should be OK.  It's now in:
    
    IOSG::USER1:[LIBRARY.A1V30]FCS.PS
