I don't know where to report Bookreader bugs,, but since its name is DECW$...
Running VMS V5.4
Bookreader:

Started bookreader from application menu in seesion manager window.
Opened a shelf.
Used the scroll bar.
Did a triple-click instead of a double-click on a book name.
Contents-list window appeared, book window appeared, bookreader crashed - dump
below.
I repeated the process twice and it crashed both times.
Doing the triple-click seems to be the problem, although it doesn't always
crash - I opened a book properly then closed it then tried the triple-click and
the book opened then closed but bookreader survived.  (It would be nice if it
didn't close immediately anyway.)
The following is what was in the Fileview-Bookreader window after the crash:
